Your privacy matters to us. This policy explains what information we collect through the Surrogacy Readiness Assessment, how we use it, and how we protect it.
When you take the assessment, we collect the answers you provide to our questions. This includes general personal information (first name, age, state of residence), health-related information (pregnancy history, height, weight, BMI, substance use), lifestyle and preference information (motivation, family support, timeline, intended parent relationship preferences), and contact information if you choose to provide it (email address and phone number). Providing contact information is optional.
We use your answers to generate your personalized readiness score and recommendations. If you provide contact information, we may use it to send you your results, share educational surrogacy resources, and follow up with you about next steps if you appear to be a strong candidate. We will never use your information for purposes unrelated to surrogacy education and support.
Your assessment responses are submitted through Netlify Forms, a secure form handling service that is SOC 2 compliant. Data is transmitted over encrypted HTTPS connections. We do not sell, rent, or trade your personal information to third parties.
We retain your assessment data for up to 12 months from the date of submission. After that period, your data is deleted. You can request deletion at any time by contacting us.
While the assessment asks about pregnancy history, BMI, and substance use, we are not a healthcare provider and this information is used solely for educational screening purposes. We do not provide medical advice, diagnoses, or treatment. The assessment is not a substitute for professional medical consultation.
The personalized insights shown during the assessment are generated by an AI language model. Your answers are sent to the AI service to generate relevant educational feedback. The AI service does not store your information beyond the immediate request.
We use two analytics services to understand how visitors use this site so we can improve it:
Google Analytics 4 measures aggregate traffic and how people move through the site (such as which articles they read before starting the assessment). Google Analytics uses cookies and may collect your IP address (anonymized), device type, browser, and pages visited. Learn more at policies.google.com/privacy. You can opt out using the Google Analytics opt-out browser add-on.
Microsoft Clarity records anonymous session replays and heatmaps so we can see where visitors get stuck. Clarity does not record values typed into form fields or passwords. Learn more at Microsoft's Privacy Statement.
Both services collect data in aggregate and are used solely to improve this site — not for advertising. We do not run advertising cookies or third-party trackers, and we do not share analytics data with advertisers.
You have the right to request access to, correction of, or deletion of your personal information at any time. If you are a California resident, you have additional rights under the California Consumer Privacy Act (CCPA), including the right to know what information we have collected and the right to opt out of the sale of personal information. We do not sell personal information.
This assessment is intended for adults aged 21 and older. We do not knowingly collect information from anyone under the age of 21.
We may update this policy from time to time. Any changes will be reflected on this page with an updated effective date.
